Understanding B6 versatility
Vitamin B6 is the cofactor for more than 150 enzymes, a record among B vitamins. In its active form PLP (pyridoxal-5-phosphate), it intervenes in the metabolism of all amino acids, the synthesis of serotonin (5-HTP to 5-HT), dopamine (L-DOPA to dopamine), GABA (glutamate to GABA), and noradrenaline. It is also essential for heme synthesis (hemoglobin), zinc absorption, and the conversion of homocysteine to cystathionine. Your good score reflects a satisfactory status of these metabolic pathways.
Populations at risk of deficiency
B6 is the most widespread B vitamin deficiency. Women on oral contraceptives are particularly exposed because synthetic estrogens accelerate tryptophan catabolism and increase PLP requirements. Alcohol, chronic stress, medications (isoniazid, penicillamine, hydralazine), and aging are other depletion factors. Remain vigilant if you belong to one of these categories.
Preserving your B6 status

Pathophysiology of your situation
PLP is the cofactor for glutamate decarboxylase (enzyme that converts excitatory glutamate to inhibitory GABA), tryptophan hydroxylase, and aromatic amino acid decarboxylase (serotonin and dopamine synthesis). PLP deficiency causes an imbalance between excitatory neurotransmitters (glutamate) and inhibitory ones (GABA), explaining spasmophilia, muscle tension, and hyperventilation. B6 is also the cofactor for ALA synthase, the first enzyme of heme synthesis: deficiency causes sideroblastic anemia with elevated iron but low hemoglobin.
Clinical and biological markers
Clinical signs according to Hertoghe include spasmophilia of the hands (Trousseau sign), hyperventilation, chronic muscle tension, angular cheilitis (fissures at mouth corners), and glossitis (painful and smooth tongue). In laboratory testing, plasma PLP assay is the reference marker (normal threshold above 30 nmol/L, optimal above 50 nmol/L). Homocysteine is often elevated (B6 is a cofactor for cystathionine beta-synthase which converts homocysteine to cystathionine). An elevated urinary xanthurenic acid to kynurenic acid ratio after tryptophan loading is a highly specific functional test.
Priority food sources
Pistachio is the most concentrated source (1.70 mg/100 g), followed by fresh garlic (1.24 mg/100 g), tuna (0.93 mg/100 g), and calf liver (0.84 mg/100 g). Sunflower seeds (0.80 mg/100 g), salmon (0.64 mg/100 g), chicken (0.60 mg/100 g), and chickpeas (0.55 mg/100 g) are accessible daily sources. Walnuts (0.54 mg/100 g) and turkey (0.46 mg/100 g) supplement intake. Banana (0.37 mg/100 g) is a practical snack rich in B6 and tryptophan.
Targeted supplementation
P5P (pyridoxal-5-phosphate) at 25 to 50 mg per day is the active form of choice. Unlike standard pyridoxine (HCl), P5P does not require liver conversion and is directly usable by enzymes. This form is particularly indicated in people with slowed liver function or genetic polymorphisms affecting conversion enzymes. Take preferably in the morning at breakfast. GABA, the brain's primary inhibitory neurotransmitter, drops first because glutamate decarboxylase has low affinity for PLP. This explains the spasmophilia, cramps, and neuromuscular hyperexcitability that dominate your clinical picture.
Priority biological assessment
Request as a priority plasma PLP (pyridoxal-5-phosphate) assay, normal threshold above 30 nmol/L, optimal above 50 nmol/L, and homocysteine measurement, normal below 10 micromol/L, ideal below 7. According to Hertoghe, major clinical signs include hand spasmophilia (positive Trousseau sign), chronic hyperventilation, painful muscle tension, bilateral angular cheilitis, and atrophic glossitis. Complete with CBC (search for microcytic or sideroblastic anemia), serum iron and ferritin assay (paradoxically elevated in sideroblastic anemia), and liver function tests. The tryptophan loading test with urinary xanthurenic acid assay is the functional gold standard.
Therapeutic nutrition

Prefer gentle cooking methods as B6 is partially destroyed by high temperatures. Combine foods rich in zinc (oysters, pumpkin seeds) to compensate for absorption deficiency linked to B6 lack.
Attack supplementation
Start with P5P (pyridoxal-5-phosphate) at 50 mg per day, the active form requiring no liver conversion. This form is 3 to 5 times more bioavailable than standard pyridoxine HCl and suits people with genetic polymorphisms (ALPL, PNPO) affecting conversion. Imperative to combine with magnesium bisglycinate (400 mg/day in 2 doses): B6 and magnesium act synergistically for GABA synthesis and neuromuscular relaxation.
